You must:
be a Registered Nurse (Level 1) or Midwife on the Nursing and Midwifery Council professional register
provide a formal declaration of good health and character
provide an up-to-date Protecting Vulnerable Groups (PVG)(Scotland) or Disclosure and Barring Service (DBS)(England, Wales, Northern Ireland) (or equivalent check)
supply a personal statement in support of your application
supply a professional reference demonstrating a level of proficiency appropriate to the degree
You must be employed (or have confirmation of an employment offer) within a practice learning environment that offers sufficient learning opportunities to enable the achievement of the standards of proficiency for your intended field of SCPHN practice (NMC 2023).
